10/23/2023 EnchantaVaughn Approved Behavior: When QMHP walked inside the home, client personality was viewed as well-mannered, courteous, and genuine. Client displayed helpful and supportive behaviors towards his mother as he assisted her with different chores (i.e. washing dishes, taking the trash out, sweeping the floor, and straightening the rooms) in the home. Client communicated pleasant and kindhearted manners during the session and assured he felt at peace. Client was in a much better mood today than his previous moods. Behavior: Client verbalized positive feelings about things he enjoyed at home, such as playing video games and talking on the phone. Client displayed good listening skills and did not have to be redirected for any behaviors at this time. Client was very attentive to the instructions and guidelines of the psycho social activity. Client demonstrated excellent conduct as he interacted well with his siblings and showed everyone in the home respect. Client was able to reduce symptoms of ADHD, as evidence by his calm demeanor. Intervention: The QMHP complemented client on his well-mannered and mature manners that he had displayed considering he didnât have much entertainment to keep him occupied or the financial means to go places like most other kids. QMHP utilized a combination of mood regulation techniques, to help client delay the need for his immediate gratification and inhibit his impulses. QMHP provided praise and encouragement to acknowledge clientâs improvements as he processed his mental competences. Intervention: QMHP identified the positive consequences client has experienced in response to delaying empathy and treating others respectfully. QMHP examined and evaluated clientâs demeanor in order to manage his compliance with following rules at home. QMHP asked client to identify, role-play, and model appropriate social skills during session. QMHP provided positive words of praise and reassurance to encourage and promote confidence and respect he is expected to display on a daily basis. Response: Client said thank you. Client was observed practicing breathing and relaxation strategies and mastering the âstop, think, actâ method. Client followed all directions, as he demonstrated ability to adhere to the rules and expectations of individual session. Client hyperactive and impulsive behaviors were reduced, and symptoms of ADHD are now stabilized. Client indicated that he was going to try his best to make changes that were important to his health. Client responded well to feedback and appeared thankful for the help and support. Response: Client was receptive and actively responded to each intervention. Client was able to use his active listening skills to pay attention and concentrate on the discussion between his mom and counselor. Client expressed positive emotions and showed respect to his counselor when asked questions about trust, love, and friendship. Client verbalized good thoughts and understanding about these topics. Client developed better communication skills by expressing himself in a calm manner.
